Here's mine in no particular order:

Use a jump cue in 14.1
Use a jump cue in 1 pocket
Airbarrel
Use sharking techniques
Cut up your backer
Dump in a tourney (especially with a calcutta)
Drill a freakin' hole in my Blue Diamond Chalk!

Feel free to add to the list.

I'm sure there are many more but placing the chalk upside down on the rail.
If you must use baby powder using so much that you leave hand prints all over the table.

I have such a bad habit of doing that.

On top of that I have a tendency to put the chalk down on the rail rather firmly leaving little blue chalk blobs :-(

IM A BAD PERSON :-(

this is just my own personal pet peeve.....dirty blue ferrules. I can't stand them

Cheating
Sharking
Air Barrel
Dumping or rigging a tourney when there's a calcutta
Whining that you didn't know the rules.
Whining that you didn't get any rolls.
People that ask you to gamble, and then start asking for a spot after the first game.

Not calling a foul on yourself

I'm not a fan of the cue extender being used in 1 pocket or 14.1.

sitting on rails or table
cue or beer in pocket
drinks on table, felt
cigarettes on rails.
cue behind neck with arms hanging on it
using cues like toys and sword fighting with'em
cigarette in mouth while at table causing ash on felt
chalking over the table
drilling paul newman style
loud a$$ wipes
people who can't shoot trying to showoff and shooting behind their back
throwing the balls on the table

Sharking
Air barrel
Failing to call a foul on yourself (or general dishonesty)
Trash talk (see #1)
Scooping/golfing the ball, or any general amateur effort at jump/masse that you shouldn't even be trying
Sitting or sprawling on the table like it's lawn furniture
Not knowing the rules/invoking house rules/being dead wrong about common-knowledge rules/arguing about calls any good player should know about such as push fouls, no rail, etc.

edit: also everything chino said. I can't stand lack of respect for the equipment.
I once removed a guy's beer from the felt (not even the rails, but the felt) and he wanted to get in a fight over it.
People are prícks.
