Willee: have you thought of adding more set screw holes in the adapter? THough I made my own adapter and made it a bit longer. And no set screws, I prefer bolts, I used 5/16 fine thread. Just long enough to make contact on my spindle, and yes I had to increase the clearance of the gear cover hole. I have six in mine, three that contact the rearmost part of my lathe spindle and the inner three close to the edge of my adapter. I use no bushing, instead I use the six bolts, and to protect my spindle I drilled out the end of each bolt and fabbed a brass button for each. I can dial in the chuck in three planes now. Works good for me. I think the last I checked I had about .001 runout. I also bored out the chuck jaws. ANd the chuck just threads on when I want to remove it, I made the threads a close fit.
