Looks like a hair tighter than 4.5, but the balls are not really in the right place to make a decent measurement. The 13 ball needs to be placed out at the very opening of the mouth (point). Then try to position the 14 in the same spot on the other facing. Looks like it will be a tight 4.5.
Edit: it might be just the pic, but it looks like the facings are void of much down angle at all.
 
Then I don't understand how pockets are measured.
The 14 ball should be placed exactly where the 13 ball is placed on the right side of the pocket - where the middle of the ball is sitting right at the the point where the cushion point is. Taking a measurement with a tape measure from point to point across the pocket gives you your pocket mouth measurement.

If it’s a 4-1/2” pocket which this looks very close to being, the middle of both balls will touch in the middle and the outside center of both balls will be touching the points.
 
Yes, it looks like the pockets/subrails could use an overhaul by a good mechanic.
New cloth and pocket facings should do the trick, assuming the PFA is no greater than 142°. Looks like the subrails may have already been extended.

Judging from the pics the PFA are much of the problem. New cloth and facings over the top of mushroomed subrail miters aren’t going to fix those pockets.
