That and dedicated practice.

Banjo strings are pretty thin. I know a banjo player. His fingers look like little knobs on the end from the calluses. Thats his left hand from fingering.

Thats a harsh picture but its not typical no matter how hard you play. I play a little banjo myself , your picking fingers don't even touch the strings. That more of a "Damn, I sliced something open but the show must go on" kinda thing. Still gutsy either way.
