You can see when he looks back to say extension, you just can't hear it. He is a low-talker, and the ref didn't hear it. But based on the replay, which they played multiple times, it's obvious where he said it.

It's like looking at the Zapruder film, but SVB's mouth doesn't move when he looks back. Regardless they should give the players the benefit of the doubt in these situations.

It looked like Shane had his hearing aid off. The clock was counting down, and when it ended the ref was right next to him and said “foul” but Shane didn’t turn at all. The ref had to go up to him and put his hand in front of him to stop him.
