Hey Dennis. If you don't mind I'd like to suggest a few things.
The first thing I noticed is you are not pausing with your tip at the cue ball. IMO this is very important. You cannot get a good line of site while sawing wood on your practice strokes.
When you are paused at the CB position, use your imagination and see the stroke going through the OB and verify in your mind where the OB is going. Then a few more practice strokes and another pause to once again verify your line of aim is on.
Also, draw an imaginary line from your CB to the OB in your mind while you are stopped at the CB with your tip. See that line and cover it with the cue shaft when you actually do stroke through your shot.
Lastly, slow smooth acceleration through the shot. You are poking and stabbing pretty hard. Even if you did aim correctly to begin with, the odds you are following that same line you aimed on are slim.
